All Brooks/UNF Residency and Fellowship programs begin in July each year and have an admissions application deadline of March 15th. Applications from candidates are welcome at any time up until the application deadline.

| 1) Write 2-page Personal Letter for Admission (PLA) |
This essay should address why you are pursuing a clinical residency experience, why you are interested in this particular residency (i.e. both the specialty area as well as with Brooks), and your professional goals subsequent to completion of this residency. Also, discuss your support and barriers that currently exist within your personal/professional life that may affect your ability to fully participate, your level of performance, and eventual successful completion of the residency program. Applicant will upload this document in Section 6: "Profile Attachments" of the Application Profile. The essay should be a maximum of two pages and a minimum of 1 page. Use the following formatting: |
| 2) Develop list of all Continuing Education courses attended within the past 5 years |
Applicant should provide the course name, date, instructor, and number of CEUs provided. Applicant will upload this document in Section 6: "Profile Attachments" of the Application Profile. |
| 3) Official Transcripts from Professional School and other Post-professional Degree Programs |
Applicants to the Residency/Fellowship Program must provide the Official Transcripts from Professional School. If the applicant is still in school, he/she may mail an unofficial transcript for the committee to review, however, if accepted he/she will not be able to start the program until Official Professional School transcripts have been received. | 4) Three (3) Professional Recommendations (names and email addresses) | You must provide 3 Professional Recommendations for consideration of the Residency/Fellowship Program. The recommendations must be from professionals within your discipline, e.g. PTs must have a PT recommendation. For student applicants, at least one of the recommendation letters should come from an academician from their professional program. Applicants will be asked to provide the Names and Email address of 3 recommendations when creating the Application Profile. A Recommendation form is automatically sent to the email addresses provided. It is the applicant’s responsibility to follow up with these individuals to ensure they have submitted a recommendation on their behalf by the March 15th due date. |
| 5) Create Application Profile | Applicants should create an Application Profile via the Brooks Rehabilitation Careers website. CLICK HERE TO ACCESS THE WEBSITE. Applicants will choose the program they wish to apply, and then select ‘New Candidate Apply.’ Here applicants will be given the opportunity to create a profile with personal information and upload documents such as a resume, cover letter, Personal Letter for Admission (PLA), Continuing Education Form, and contact information for Professional Recommendations. |
| 6) Complete Job Application | Once the Application Profile is completed, qualified applicants will be emailed a link to complete the Job Application. This application drills down even further into the applicant’s education and employment history. Here applicants will be asked to provide information such as start and end dates, references, and the reason for leaving each position. |
| 7) News of Acceptance/Denial to Program | All applicants will be informed of acceptance/denial to the program by Friday, May 18th. |
