Frequently Asked Questions for the Brooks Balance Center

How do I become a patient?
In order for you to be evaluated and treated at the Brooks Balance Center, we require a referral from your physician. This can be from your primary care physician or a specialist physician. They can fax the referral directly to us at 904-854-2058 or they can give you the referral to call us at 904-854-2050 

What should I expect each visit?
On the first visit you will be evaluated by a specially trained physical therapist. Prior to being evaluated, you will need to fill out brief paperwork about your medical history and insurance information. The first day takes approximately 45-60 minutes, so we ask that you arrive before your scheduled appointment to complete the necessary paperwork. Subsequent visits will also be 45-60 minutes which will be geared toward patient education on specific exercises to your diagnosis and specialized exercises unique to your impairments and functional limitations. A typical course of therapy can range from 2-3 visits up to 8 weeks depending on your diagnosis. Following your evaluation, the therapist will discuss the expected length of your treatment as well as the findings on the evaluation and goals of rehabilitation.

What should I wear?
Comfortable shoes without heels are preferred. Clothing which allows you to move and walk freely without restrictions.

What insurances are accepted?
We take most insurance with the exception of Medicaid programs. We will verify coverage prior to your initial visit to ensure your plan covers our services
