Medical Director
Mark Hofmann, M.D. is the pain program medical director. Dr. Hofmann has been an attending physician at Brooks Rehabilitation Hospital since 1991 and the pain program medical director since 1999. He graduated with high honors from the University of California, in Davis, California. In May, 1987 he graduated from the St. Louis University School of Medicine. His residency was completed at the Medical College of Wisconsin in 1991, where he was Chief Resident in 1990. He has been a licensed physician in Florida since 1991 and has been a Diplomat of the American Board of Physical Medicine and Rehabilitation since 1992. He is certified by the American Board of Independent Medical Examiners and the American Board of Pain Medicine. He also maintains his own private practice and is available to provide continued medical management (if needed) when patients complete the program.
Licensed Psychologist
Dr. Wittmer has a Ph.D. in clinical psychology from Washington State University, and did his internship at The Ohio State University Medical Center. His primary specialty area of study at Washington State was in health psychology, which applies the principles of cognitive restructuring and behavior modification to patients with primary medical disorders such as obesity, diabetes, cardiac disease, cancer, chronic pain, physical disabilities, and smoking cessation.
Dr. Wittmer is a licensed psychologist in the State of Florida. He began his training and experience with chronic pain patients in 1982. He has been at Brooks Rehabilitation for the past 22 years in the dual position of clinical psychologist and Executive Director of Brooks Behavioral Medicine (which includes the pain rehabilitation program).
He has published articles on chronic pain for years in his association (as courtesy professor in the Clinical and Health Psychology Department) with the University of Florida. Brooks Health System and the University of Florida developed the Brooks Center for Rehabilitation Studies (BCRS) in order to promote research for people with disabilities. Dr. Wittmer is actively involved with research through BCRS.
Biofeedback Therapist
Stephen specializes and provides biofeedback therapy for chronic pain syndromes, headaches and other stress related disorders. He received his Bachelor and Master’s degree in Allied Health Science at the University of North Florida. He completed the Biofeedback Special Studies Program at the University of North Florida that prepared him for national certification by the Biofeedback Certification Institute of America and he is a Senior Fellow.
He is a member of the national organization the Association for Applied Psychophysiology and Biofeedback and the American Pain Society. He has pioneered biofeedback as a clinical application since 1978-79 and is a founding member of three predecessors Stress Management and Health Services, Inc., Fullife, Inc., and the Southeastern Pain Clinic. He has developed programs and instructed healthcare professionals in the foundations, advanced knowledge and skills in the utilization of biofeedback techniques.
He was also an instructor of Stress Management for the Florida Jr. College at Jacksonville. He is a Vietnam Era veteran of the United States Air Force, specializing in automatic flight control systems and was nominated to President Reagan for special recognition.
Licensed Clinical Social Worker
Mary Ann specializes in providing psychotherapy services to help individuals deal more effectively with the physical and emotional impact of chronic pain in their lives. The goal is to assist individuals with improving pain, mood, and their function by a variety of cognitive and behavioral strategies for managing chronic pain.
She has a Master’s degree in Health Administration from the University of North Florida and a Master’s degree in Clinical Social Work from Florida State University. She is a Licensed Clinical Social Worker in the State of Florida and she is Board Certified in Clinical Social Work. She holds membership in the American College of Healthcare Executives, Upsilon Phi Delta, National Association of Social Workers, and the Academy of Certified Social Workers.
She is on staff as a part time adjunct instructor at the University of North Florida, Brooks College of Health, Department of Health Administration. She is fluent in American Sign Language for the Deaf and the Hard-of-Hearing.
Physical Therapist
Dr. Davis has been practicing since 1985. Although she has treated a variety of orthopedic conditions, she has specialized in chronic pain since 1990. She received her BS in Physical Therapy from the Medical College of Georgia, an MS in management through Troy State University, and her doctorate in Physical Therapy from Marymount University. She completed the Brooks Orthopedic Residency in 2007. Dr Davis is certified as an Ergonomic Assessment Specialist and is recognized as a Clinical Associate of pain in the American Academy of Pain Management. She holds membership in the American Physical Therapy Association, the Orthopedic Section and Pain Management Special Interest Group. As a member of the Florida chapter of the Physical Therapy Association she is in the process of developing a Pain Management Special Interest Group in the state of Florida. Dr Davis is an active member of the Reflex Sympathetic Dystrophy Association and periodically writes for their newsletter. She occasionally adjuncts part time as a professor for the Physical Therapy Department at the University of North Florida.
Case Manager
Amanda is dedicated to Rehabilitation Nursing and is committed to caring for people with physical disabilities and chronic pain. She has been the program case manager / nurse since 1997. She attended Florida State University in Tallahassee, FL and graduated with an A.S. in Nursing from Florida Junior College in Jacksonville, FL. She has been licensed as a Registered Nurse in Florida since 1979 and has worked in multiple healthcare settings. In 1990, she became a Rehabilitation Provider with the Division of Workers Compensation and assisted injured workers with medical case management and vocational services. Amanda has maintained certification as a rehabilitation registered nurse (CRRN) through the Rehabilitation Nursing Certification Board since 1992 and maintained certification as a case manager (CCM) with the Certification of Insurance Rehabilitation Specialists Commission / Commission for Case Manager Certification since 1993.
Physical Therapist
Joshua Schuette has been specializing in the field of chronic pain since 2005 when he graduated from the University of St. Augustine for Health Sciences with his Doctorate of Physical Therapy degree. Prior to working at Brooks Rehabilitation, Joshua was the Director of Physical Therapy at the National Pain Institute in Winter Park, Florida. Before working in the field of physical therapy, Dr. Schuette educated and worked with children and adults with disabilities as an adapted physical education teacher. He also has prior experience in working in interdisciplinary settings as a physical education instructor for the Multidisciplinary Diagnostic and Training Program, an education program conjointly run by the Colleges of Medicine and Education at The University of Florida.
Dr. Schuette’s educational background also includes a Bachelor’s and Master’s degree in Exercise and Sport Sciences from the University of Florida. He is currently working on a Master of Business Administration degree at the University of North Florida. He is also a Certified Strength and Conditioning Specialist and Certified Personal Trainer through the National Strength and Conditioning Association.
