Many patients about to enter Brooks Outpatient Rehabilitation want to know what they can expect from their experience. We want to provide you with some answers so you will be prepared to jumpstart your recovery and get back to being you again.

What do I need to begin therapy?
You will need a referral from your physician. Once you have a referral, call Brooks Outpatient Rehabilitation at (888) 323-8005 to set up an appointment at the center nearest you.

How many visits am I allowed for therapy?
This depends on both your insurance coverage and your medical necessity. We can help you determine what your insurance carrier will cover and any costs you may incur. Your therapist will also discuss with you the expectations for your recovery and will help you set goals and timelines.

How long will my evaluation take?
Evaluations range from 30-60 minutes depending on the diagnosis and the type of problem. Patients with acute injuries may not require or be able to tolerate extensive testing. Some chronic and complicated conditions may require more thorough evaluation.

What should I bring to my therapy sessions?
On your first day of therapy, remember to bring your insurance information, any paper referrals, insurance pre-authorization, prescriptions and any copies of diagnostic tests or procedures. It’s also helpful to know the names of any medications you are taking.

What should I wear to therapy?
Loose-fitting, comfortable clothing and shoes are most appropriate for therapy sessions. In addition, make sure to wear or bring any braces or splints that you have.

Medical history
Before you arrive at Brooks, please take the time to print out and complete our online Medical History form, and if applicable, the Medicare Secondary Payor form. This allows us to identify any preconditions that may prevent you from being able to receive rehabilitation treatment from us. It also allows us to begin creating an individualized treatment plan and recovery goals for you immediately upon the start of your outpatient rehabilitation at Brooks.
