Your case manager will help determine when you are ready to be discharged to your home or the next stage of your recovery, based on the recommendations of your physicians and therapists. If you are not able to return to your home, your case manager will help you to transition into the next level of care that you require.

Outpatient care is essential for many patients following their stay at Brooks Rehabilitation Hospital. Our extensive network of outpatient clinics is designed to meet your therapy needs with services for patients of all ages. All treatment disciplines are available, including physical, occupational, speech-language and cognitive therapies.
Many patients attend hour-long therapy sessions three days a week, and our large network of rehabilitation centers makes it easy to find a convenient location.
