Getting a Referral

All of our patients must obtain a physician referral before being admitted to Brooks Rehabilitation Hospital or receiving treatment at any of our Brooks Outpatient locations. Contact your doctor to determine if Brooks is right for you.

Once you have your referral, please contact us to get started. We will need the patient's demographic information—address, date of birth, next of kin—and insurance information. Our rehabilitation services are covered by most insurance policies, and you can see a complete list here. If appropriate, financial assistance plans can also be discussed.
